AUTOPAY CONTRACT

By signing onto AutoPay, I authorize Altitude Fitness Whistler LTD. to charge my Credit Card or Debit my Bank Account for $98 (including tax) every 4 weeks (28 days). The first payment will be $130 (including tax), which includes the $32 (including tax) sign-up fee.

Terms & Conditions:

I. Cancellation Policy:

• This plan may be canceled at any time with no fees by emailing: hi@altitudecanada.com at least 48 hours before the next payment is set to come out. All payments are final (See Section IV).

II. Hold Policy:

• This plan may be placed on hold at any time by emailing: hi@altitudecanada.com. Pass Holds are subject to a $20(including tax) fee. This action will preserve any remaining days on the pass and reset the payment schedule. I acknowledge that Pass Holds are only valid for 1 year from the purchase date, with an option to purchase a $20(including tax) Pass Hold Extension at the end of 1 year if the AutoPay membership is still inactive.

III. Payment Failures:

• Failed automatic payments may be subject to a $5.00 (including tax) fee.

• The membership may be terminated due to non-payment (See Section V).

IV. Refunds, Transfers and Price Adjustments:

• I acknowledge that this pass is non-refundable, non-transferable and the monthly automatic payment amount is subject to change. All active AutoPay members will receive an email with 28 days' notice regarding any price increases to their membership.

V. Termination of Membership:

• To terminate the membership, all payments must be settled. If the account is terminated due to non-payment, reactivation will require settlement of the outstanding balance, a new 4-week membership fee, and enrollment fee.
